Does osteopathy hurt? Soreness after treatment explained

What osteopathic treatment usually feels like, why some people feel sore afterwards, how long it lasts, and when to get advice.

Most people find osteopathic treatment comfortable, and the techniques are adapted to you. Some techniques, such as working into tight muscles, can feel tender, and some people feel sore or tired for a day or two afterwards. You stay in control throughout, and you can ask the osteopath to stop or to change what they are doing at any point.

What it usually feels like

Treatment can include soft-tissue work, gentle stretching and joint mobilisation, and, where appropriate and with your consent, quicker techniques that may make a popping sound. Not every treatment involves clicking or 'cracking', and the sound is not a bone going back into place. You can say if something is uncomfortable.

Soreness afterwards

Reviews of spinal manipulative therapy trials report that the common side effects are mostly musculoskeletal, such as muscle soreness, stiffness or a temporary increase in pain, usually mild to moderate and short-lived. Some people also feel tired, and in practice many people feel a little sore for a day or two afterwards.

What can help afterwards

  • Keep moving gently, and avoid anything that clearly aggravates it for a day or so
  • Warmth, such as a warm bath or shower, can feel soothing
  • Drink water, and rest if you are tired
  • Do any exercises you have been given, unless you were told otherwise

When to get advice

  • If your pain is significantly worse and does not settle after a day or two, contact your osteopath or GP
  • If you develop new numbness, weakness, severe headache, dizziness, chest pain or changes in bladder or bowel control, seek urgent medical advice. Call 999 for chest pain, signs of a stroke, or loss of bladder or bowel control

Common questions

Is it normal to be sore after seeing an osteopath?

Yes. Soreness, stiffness or a temporary increase in pain for a day or two is the most commonly reported mild effect of manual therapy. If pain is significantly worse and does not settle, or you have new symptoms, get advice.

Will the osteopath crack my back?

Only if it is appropriate and you agree to it. Many techniques involve no clicking at all, and the sound, when it happens, is gas moving in the joint fluid, not a bone going back into place.
